What are Neuromodulators?

Botox (Allergan), Dysport (Galderma), and Xeomin (Merz) are all injectable neuromodulators, meaning they can temporarily alter the nerve impulses of targeted muscles, therefore reducing or eliminating contraction-induced wrinkles.

Each brand is a slightly different formulation of the same substance, Botulinum Toxin A (BTA), but they all work the same way: they block the nerve receptors of the injected muscles to temporarily paralyze muscle movements that cause wrinkles. The most common targets are horizontal lines across the forehead and perpendicular frown lines between the brows, but many also rely on Botox, Dysport, and Xeomin for crow’s feet, nasal squint lines, lines around the neck, mouth and chin, eyebrow lifting, and even profuse underarm sweating and migraines.

A toxin is a toxin is a toxin. So…What’s the difference?

Even though doctors have been using Botox for cosmetic use since the ’50s and ’60s, it was only in 2002 that Botox received FDA approval for treating frown lines and facial wrinkles. Doctors had been using the drug to treat forehead lines “off-label,” meaning outside of its FDA-approved use, for decades. Once a drug obtains FDA-approval for one condition, a licensed physician can legally prescribe it for other conditions that they believe can benefit from the drug. The recent FDA-approval for the use of Botox for forehead lines, crow’s-feet and glabellar lines, just means that the drug has passed numerous clinical trials and is proven to do what doctors and patients around to the world already know it can, temporarily banish lines and wrinkles. Botox enjoyed decades as the only drug of it’s kind, making it certainly the most well known and therefore, the most widely used.

    In 2009, Dysport got it’s FDA approval and made its way into the market. Primarily used in Europe until recently, Dysport use in the United States has grown in leaps and bounds over the past few years. The main difference between Dysport and Botox is that Dysport contains smaller molecules, typically leading to faster results. You may notice facial changes in three to five days with Dysport versus the normal seven with Botox. Because of its size, Dysport can be more useful for larger treatment areas as it diffuses more and creates a subtler, more natural effect. Because Dysport molecules are smaller, the units are calculated differently than Botox and Xeomin, which are measures identically. Generally, a provider will use 3 units of Dysport for every 1 unit of Botox or Xeomin. The per unit cost is usually 1/3 to reflect this difference. The treatment lasts just as long as Botox and Xeomin.

      Xeomin is the latest addition to the BTA family. It was approved by the FDA in 2011. Xeomin is a little different from both Botox and Dysport. It is the purest form of BTA. According to The Journal of Clinical and Aesthetic Dermatology in February 2014: “Xeomin is free of therapeutically superfluous complexing proteins; its purer formulation has been suggested to lead to greater efficacy”. This means that Xeomin does not have the unnecessary protein ‘shell’ that both Dysport and Botox have (imagine an M&M without the coating). Your body does not need to break down that coating and as such, the BTA works more effectively. Though it uses the same toxin, Xeomin also includes no additives, meaning that there is less risk of your body forming antibodies to resist it. Some patients have more difficulty getting results using Botox or Dysport over time – that won’t happen with Xeomin. It also has the least chance of inducing an allergic reaction. You could see results from your treatment in as little as three days and results last the same as the other two.
