PT-141 Nasal Spray vs. Injection: What's the Difference?

Most conversations about PT-141 (bremelanotide) focus on the injectable form, but at our Knoxville practice, we’re asked just as often about the nasal spray version — how it compares, whether it works as well, and which one might be the better fit for a given patient.

Quick Answer: Both forms of PT-141 work through the same mechanism, but they differ in onset, duration, and side effect profile. The nasal spray tends to act a bit faster (often within 30–60 minutes) but doesn’t last as long (roughly 2 hours), while the injectable form typically takes somewhat longer to reach full effect but lasts considerably longer (commonly cited up to 8–12 hours) and tends to produce a stronger overall response, sometimes with slightly more side effects.

PT-141 works by acting on melanocortin receptors in the brain to increase sexual desire and support erectile function — a different mechanism than PDE5 inhibitors like Viagra or Cialis, which work on blood flow. Both the nasal spray and injectable forms deliver the same active medication, just through different routes:
